Consumer Information and Regulator Disclosure.

SUBSTANCE ABUSE POLICY

1. Purpose

Health Tech Associates is committed to maintaining a safe, healthy, and drug-free educational and work environment. In compliance with Iowa Code § 256.183(1)(e) and Iowa Administrative Rule § 283-21.3(5), the unlawful possession, use, manufacture, or distribution of controlled substances or alcohol is strictly prohibited on school premises and during any school-sponsored activities.

This policy applies to all students and employees of Health Tech Associates.

2. Prohibited Conduct

The following conduct is strictly prohibited:

  • Unlawful possession, use, sale, manufacture, or distribution of controlled substances.
  • Possession or use of alcohol on school premises or during school-sponsored activities.
  • Reporting to class, laboratory, clinical, or work under the influence of illegal drugs or alcohol.
  • Misuse or diversion of legally prescribed medications.

“Controlled substances” are defined as those prohibited under federal law and the laws of the State of Iowa, except when used pursuant to a valid prescription and in accordance with medical direction.

3. Sanctions for Violation

Violations of this policy may result in disciplinary action up to and including dismissal from the educational program or termination of employment.

Students

Disciplinary action may include:

  • Written warning and documented disciplinary action
  • Temporary suspension from class, laboratory, or clinical activities
  • Required withdrawal or dismissal from the program
  • Notification to clinical and/ or educational partners, when applicable

Employees

Disciplinary action may include:

  • Written warning
  • Suspension
  • Termination of employment

Violations may also be referred to appropriate law enforcement authorities when warranted.

4. Legal Consequences and Health Risks

Unlawful possession or distribution of controlled substances may result in criminal penalties under federal and Iowa law, including fines and imprisonment.

Substance abuse poses significant health risks, including but not limited to:

  • Physical dependence and addiction
  • Organ damage
  • Impaired judgment and cognitive function
  • Increased risk of injury or death
  • Long-term mental health complications

As a healthcare training institution, Health Tech Associates emphasizes that impairment compromises patient safety, professional integrity, and future employment eligibility within healthcare settings.

5. Counseling, Rehabilitation, and Assistance

Health Tech Associates does not provide on-site counseling or employee assistance services. However, if a student or employee discloses a need for substance abuse counseling, rehabilitation, or assistance, Health Tech Associates will assist the individual in identifying and contacting appropriate local, state, or national resources.

6. Prevention Information and Resources

Health Tech Associates makes substance abuse prevention information and referral resources available digitally through both:

  • The company portal
  • The Health Tech Associates website under Policies and Procedures

Prevention resources include:

  • Links to national and state treatment locator services
  • Public educational materials provided by SAMHSA and Iowa HHS
  • Information regarding the legal consequences and health risks associated with substance abuse

7. Policy Disclosure

The full Substance Abuse Policy is publicly posted on the Health Tech Associates website under the Policies and Procedures section.

In addition:

  • The policy is maintained in the company portal for digital access by employees.
  • A synopsis of the policy is included in the Student Handbook.
  • Employees are informed that institutional policies, including this Substance Abuse Policy, are available online.
  • Printed copies are available upon request from administration.

The policy remains accessible at all times for review.

SEXUAL MISCONDUCT AND SEXUAL ABUSE POLICY

Health Tech Associates

1. Purpose

Health Tech Associates is committed to maintaining a safe, respectful, and professional educational and work environment. In compliance with Iowa Code § 256.183(1)(f) and Iowa Administrative Rule § 283-21.3(5), this policy establishes procedures for prompt reporting of sexual abuse or sexual misconduct and provides access to educational and counseling resources or referrals to local, state, and national support services.

This policy applies to all students and employees.

2. Prohibited Conduct

Health Tech Associates prohibits sexual misconduct in any form, including but not limited to:

  • Sexual abuse
  • Sexual assault
  • Sexual harassment
  • Unwelcome sexual advances
  • Requests for sexual favors
  • Verbal, physical, or visual conduct of a sexual nature that creates an intimidating, hostile, or offensive environment
  • Retaliation against any individual who reports sexual misconduct

Sexual misconduct is prohibited on school premises, during school-sponsored activities, and in clinical settings affiliated with Health Tech Associates.

3. Prompt Reporting Procedures

Health Tech Associates encourages immediate reporting of sexual misconduct.

Reports may be made to:

  • The School Administrator
  • A direct supervisor (for employees)

Reports may be made verbally or in writing.

Upon receiving a report, Health Tech Associates will:

  • Respond promptly and take the complaint seriously
  • Assess immediate safety concerns
  • Take appropriate interim protective measures when necessary
  • Conduct an internal review appropriate to the circumstances
  • Refer the matter to local law enforcement when required or requested

Retaliation against any individual who reports sexual misconduct or participates in an investigation is strictly prohibited.

4. Security and Law Enforcement

Health Tech Associates does not maintain a campus police department or separate campus security personnel.

In the event of an emergency, individuals should immediately dial 911.

Local law enforcement serves as the primary security authority for criminal matters. Health Tech Associates will cooperate fully with law enforcement investigations as appropriate.

5. Educational, Counseling, and Support Resources

Health Tech Associates does not provide on-site counseling services. However, individuals affected by sexual abuse or misconduct will be assisted in identifying and contacting appropriate local, state, and national educational and counseling resources.

6. Confidentiality

Health Tech Associates will make reasonable efforts to protect the privacy of individuals involved in a report. However, confidentiality cannot be guaranteed when disclosure is necessary to ensure safety or comply with legal obligations.

7. Policy Disclosure

In addition:

CHILD AND DEPENDENT ADULT ABUSE REPORTING POLICY

Health Tech Associates

1. Purpose

Health Tech Associates is committed to protecting vulnerable populations and complying with applicable Iowa law. In accordance with Iowa Code § 256.183(1)(h), Iowa Administrative Rule 283-21.3(5)(b), and applicable Iowa child and dependent adult abuse reporting laws, this policy establishes reporting responsibilities for employees who, in the course of their professional duties, suspect:

  • Child physical or sexual abuse; or
  • Dependent adult abuse.

This policy applies to all employees of Health Tech Associates.

2. Definitions

For purposes of this policy:

Child – An individual under eighteen (18) years of age.

Dependent Adult – An individual eighteen (18) years of age or older who is unable to protect their own interests or adequately perform essential daily tasks due to physical or mental impairment.

Abuse – Includes physical abuse, sexual abuse, exploitation, neglect, or other conduct defined as abuse under Iowa law.

3. Circumstances Requiring Reporting

An employee must report when, in the course of their employment responsibilities, they:

  • Observe signs of physical injury inconsistent with explanation;
  • Receive a disclosure from a child or dependent adult regarding abuse;
  • Witness behavior that reasonably suggests abuse;
  • Have reasonable cause to suspect that abuse has occurred.

Proof or confirmation is not required. Reasonable suspicion is sufficient to trigger reporting.

4. Timeframe for Reporting

An employee who suspects abuse shall report the suspected abuse immediately, and no later than twenty-four (24) hours after forming the suspicion.

5. Method of Reporting

When suspicion arises, the employee shall:

Step 1 – Notify School Administration

Immediately notify the School Administrator.

Step 2 – Report to Authorities

For suspected child abuse, contact:

For suspected dependent adult abuse, contact:

Iowa Dependent Adult Abuse Hotline (24/7): 1-800-362-2178

If the individual is in immediate danger, call 911.

Reporting to school administration does not replace the requirement to report directly to the appropriate state hotline or law enforcement.

6. Good Faith Reporting

Nothing in this policy shall prohibit or discourage an employee from reporting suspected abuse in good faith directly to law enforcement.

Employees who report in good faith shall not be subject to retaliation.

7. Confidentiality

Reports will be handled with discretion and shared only as required by law or necessary to ensure safety.

8. Policy Disclosure

This Child and Dependent Adult Abuse Reporting Policy applies to all employees.

The policy is:

  • Included in the Employee Handbook;
  • Accessible through the company portal;
  • Publicly posted on the Health Tech Associates website under Policies and Procedures;
  • Available in printed form upon request.

Employees are responsible for reviewing and complying with this policy as a condition of employment.

MILITARY TUITION AND FEE REFUND POLICY

Health Tech Associates

1. Purpose

In compliance with Iowa Code § 256.183(1)(g) and Iowa Administrative Rule § 283-21.3(5), Health Tech Associates has adopted the following Military Tuition and Fee Refund Policy for eligible students who are called to military service.

This policy applies to a student who:

  • Is a member of the Iowa National Guard or reserve forces of the United States; or
  • Is the spouse of such a member if the member has a dependent child;

and who is ordered to national guard duty or federal active duty.

2. Student Options Upon Activation

An eligible student who is ordered to active duty may choose one of the following options, subject to academic feasibility, regulatory requirements, and program design:

Option A – Full Withdrawal and Full Refund

The student may withdraw from the student’s entire registration and receive a full refund of tuition and mandatory fees for the affected payment period.

Option B – Maintain Full Registration (When Academically Feasible)

The student may request to make arrangements with all instructors for:

  • Course grades; or
  • Incompletes to be completed at a later date.

If such arrangements are approved:

  • The student’s registration shall remain intact; and
  • Tuition and mandatory fees shall be assessed in full for those courses.

However, due to the clock-hour structure, cohort design, clinical requirements, or state regulatory standards applicable to certain programs, it may not be academically feasible to grant an incomplete or preserve registration. In such cases, the student may be required to restart the course or program at a later date without academic penalty.

Option C – Partial Course Arrangements (When Academically Feasible)

The student may request to make arrangements with some instructors for:

  • Course grades; or
  • Incompletes to be completed at a later date.

If arrangements are approved for specific courses:

  • Registration for those courses shall remain intact; and
  • Tuition and mandatory fees shall be assessed for those courses.

Any course for which arrangements cannot be made due to academic, clinical, or regulatory requirements shall be considered dropped, and tuition and mandatory fees for that course shall be refunded.

Students required to restart a course due to activation shall be permitted to re-enroll in a future offering without academic penalty.

3. Clock-Hour Program Provision

For programs in which academic progress is measured in clock hours, Health Tech Associates shall provide a full refund of tuition and mandatory fees for the payment period in which the student withdraws, if the student elects withdrawal under this policy.

The payment period shall be determined in accordance with rules promulgated by the U.S. Department of Education for the disbursement of Federal Stafford Loan Funds, where applicable.

4. Documentation Requirement

Students requesting benefits under this policy must provide a copy of official military activation orders or other appropriate documentation verifying eligibility.

5. Request Procedure

A student seeking relief under this policy must:

  1. Notify the School Administrator as soon as reasonably possible; and
  2. Submit documentation of activation orders.

Health Tech Associates will work promptly with the student to determine the selected option and implement the appropriate refund or academic arrangements.

Health Tech Associates will make reasonable efforts to accommodate activated students in a manner that balances statutory protections with program integrity and regulatory compliance requirements.

6. Policy Disclosure

The Military Tuition and Fee Refund Policy is publicly posted on the Health Tech Associates website under the Policies and Procedures section.

In addition:

  • The policy is included in the Student Handbook;
  • It is accessible through the company portal; and
  • Printed copies are available upon request.

STUDENT ACADEMIC RECORD SAFEGUARDING AND RETENTION POLICY

Health Tech Associates

1. Purpose

Health Tech Associates is committed to safeguarding student academic records to prevent destruction, unauthorized disclosure, loss, or theft. This policy outlines the institution’s procedures for secure storage, electronic safeguards, disaster recovery, backup processes, and long-term transcript preservation, including contingency planning in the event of school closure.

2. Records Maintained

Health Tech Associates maintains academic records including:

  • Enrollment agreements
  • Attendance documentation
  • Grades and competency evaluations
  • Skills checklists
  • Clinical documentation (where applicable)
  • Completion records
  • Transcripts
  • State testing eligibility documentation
  • Tuition and payment records

3. Physical Record Security

When paper records are maintained:

  • Records are stored in locked filing cabinets within secured administrative offices.
  • Offices remain locked when unattended.
  • Access is limited to authorized administrative personnel.
  • Paper records eligible for disposal are securely shredded.

4. Electronic Record Security

Student academic records are maintained electronically using secure, password-protected systems.

Health Tech Associates utilizes:

  • Password-protected computers for all staff access
  • Unique user credentials for each authorized employee
  • Role-based access permissions within Monday.com CRM
  • Cloud-based storage systems with secure login authentication
  • Access restricted to staff with legitimate educational or administrative responsibilities

Access to student records is limited to authorized personnel only.

5. Backup Procedures

Student academic records maintained within Monday.com CRM and related cloud-based systems are stored on secure cloud servers maintained by the service provider.

Health Tech Associates relies on the cloud provider’s data security and backup systems to preserve electronic records. Administrative oversight ensures continued access and integrity of student records.

When appropriate, periodic exports or secure downloads of essential academic data may be maintained for redundancy.

6. Disaster Recovery

In the event of fire, natural disaster, theft, hardware failure, or other catastrophic event:

  • Cloud-based systems allow continued access to student academic records from secure devices.
  • Physical records, if damaged, are reconstructed using electronic records where available.
  • Password-protected remote access supports operational continuity.

Because academic records are maintained electronically in a cloud-based system, the risk of total record loss is minimized.

7. Record Retention

Health Tech Associates retains student academic records as follows:

  • Permanent academic records and transcripts: Retained indefinitely
  • Enrollment agreements: Minimum five (5) years
  • Attendance and grade documentation: Minimum five (5) years
  • Tuition and financial records: Minimum five (5) years
  • Records required by state or federal regulation: Maintained in accordance with applicable requirements

8. Transcript Preservation in the Event of School Closure

In the event Health Tech Associates ceases operation:

  • Permanent academic records, including transcripts, will be preserved.
  • Electronic academic records maintained in Monday.com CRM will be securely exported and transferred to an appropriate third-party records custodian or other designated entity in accordance with Iowa regulatory requirements.
  • Students will be notified regarding how to request transcripts from the designated custodian.
  • The institution will comply with directives from applicable Iowa authorities regarding record preservation and access.

9. Confidentiality

Health Tech Associates protects student academic records from unauthorized access or disclosure. Employees who access or disclose student records without authorization may be subject to disciplinary action.

DISASTER RECOVERY ADDENDUM

Health Tech Associates

Health Tech Associates maintains procedures to ensure the preservation and security of student academic records in the event of disaster, system failure, or business interruption.

Electronic Records

Student academic records are maintained in secure, cloud-based systems, including Monday.com CRM, accessed through password-protected computers. Because records are stored electronically in a cloud-based environment, data remains accessible from alternate secure devices if local hardware is damaged or the facility becomes inaccessible.

Physical Records

Paper records, when maintained, are stored in locked filing cabinets within secured administrative offices. Electronic records serve as the primary record source to minimize risk of loss.

Backup and Data Protection

Health Tech Associates relies on the data security and backup infrastructure of its cloud service providers to preserve student records. Administrative oversight ensures continued access and data integrity.

Business Interruption

In the event of temporary closure due to disaster or emergency, administrative operations, including transcript access, may continue remotely.

Permanent Record Preservation and Closure Plan

Permanent academic records, including transcripts, are retained indefinitely. In the event Health Tech Associates ceases operation, electronic academic records will be securely exported and transferred to an appropriate third-party custodian or designated authority in accordance with Iowa regulatory requirements to ensure continued student access.
